Stubborn pockets of fat that don't shift with diet or exercise, a double chin that persists despite an otherwise healthy weight, love handles that outlast every gym membership, are among the most common aesthetic frustrations I hear about. Fat-dissolving injections, most commonly recognised under the brand name Aqualyx, have become one of the more talked-about non-surgical options for this specific problem.
They're also widely misunderstood, partly because the market now includes both a genuinely regulated, well-studied product and a growing number of unregulated alternatives being sold under similar-sounding names. This article sets out how Aqualyx actually works, what a realistic course of treatment looks like, and what's worth knowing before you book.
Aqualyx is a CE-marked injectable solution based on a modified form of deoxycholic acid, a bile acid your body already produces to help digest fat in the gut. Developed in Italy and introduced to the UK in 2013, it's supplied through a regulated pharmacy distribution chain and, in practice, is treated as a restricted product only available through a prescriber or appropriately qualified clinician, rather than something anyone can simply order and administer.
It belongs to the same broad drug class as deoxycholic acid injections approved in the United States and Canada under the brand names Kybella and Belkyra, though, as covered in more detail below, this isn't quite the same thing as saying Aqualyx itself has been through identical regulatory approval in those markets.
It's also worth understanding what Aqualyx is not. It isn't a filler, since it removes tissue rather than adding it, and it isn't a muscle-relaxing treatment like Botox. It sits in its own category, sometimes referred to as injection lipolysis, alongside a small number of other deoxycholic acid-based products with varying levels of regulatory approval and published evidence.
The mechanism is fairly straightforward once you understand what deoxycholic acid does. When injected into a pocket of subcutaneous fat, it disrupts the outer membrane of nearby fat cells (adipocytes), causing the cell to break down, a process called adipocytolysis. The cell's stored fat content is released into the surrounding tissue, and your body's own immune cells, mainly macrophages, gradually clear away the cellular debris and released fat over the following weeks via the lymphatic system.
Because the fat cells themselves are destroyed rather than simply shrunk, the reduction in that specific area is considered long-lasting, provided your overall weight remains stable. It's worth being clear that this doesn't stop your body storing fat elsewhere if your weight increases; the treated cells are gone, but the underlying biology of weight gain and loss is unchanged.
Aqualyx is used across a range of areas, though the strength of the evidence and how predictable the results are varies by location:
It should never be injected close to the eyes or eyelids, an area sometimes requested for under-eye fat pockets, since the skin and underlying structures there are too delicate and the safety profile hasn't been established for this use.
How much any given area responds also depends on the type of fat involved. Aqualyx works on subcutaneous fat, the layer sitting just beneath the skin, rather than visceral fat around internal organs, which is why it has no effect on abdominal fat linked to overall weight rather than a discrete, pinchable pocket. This is a genuinely useful way to think about candidacy: if you can pinch the area and feel a defined, localised bulge, it's a more promising candidate than a general area of overall fullness.
This is worth being precise about, because it's often glossed over. The strongest clinical trial evidence for deoxycholic acid as a fat-dissolving injectable comes from the phase III REFINE trials, which tested ATX-101, the specific formulation approved as Kybella in the US and Belkyra in Canada, for submental fat. These trials found that significantly more people achieved a meaningful improvement in chin profile with ATX-101 than with placebo, with the main side effects being swelling, bruising, pain and numbness at the injection site.
Aqualyx shares the same active mechanism, deoxycholic acid causing adipocytolysis, and has its own separate body of published data and over a decade of UK clinical use, but it is a distinct product from ATX-101, developed and manufactured differently, and hasn't gone through identical FDA trials in the US. This isn't a reason to dismiss Aqualyx, but it's a more accurate picture than treating the two names as interchangeable, or assuming every study of one automatically applies to the other.
A typical Aqualyx appointment follows a fairly consistent pattern:
There's genuinely no significant downtime in the sense of needing time off work, but the swelling in the first few days, especially after chin treatment, can be more noticeable than people expect from photos they've seen online, and it's worth planning around this if you have events or photos planned shortly after treatment.
If you're carrying significant excess weight rather than a small, stubborn pocket of fat, a more appropriate starting point is medically supervised weight loss support, since Aqualyx isn't designed or evidenced for that purpose.
This distinction has become genuinely important. In April 2025, the General Pharmaceutical Council issued a formal warning about pharmacies and practitioners supplying unlicensed, unregulated cosmetic injectable products marketed under names such as “Lemon Bottle,” stating that such products have “potentially caused serious harm.” These products are not the same as Aqualyx, are not MHRA-licensed or CE-marked in the same way, and generally lack comparable published safety data.
If you're considering a fat-dissolving injection, it's genuinely worth asking your practitioner exactly which product they're using, whether it's CE-marked, and whether they can show evidence of a regulated supply chain, rather than assuming all “fat dissolving injections” marketed online are equivalent.
As with other injectable treatments covered in this series, non-surgical cosmetic procedures in the UK remain less tightly regulated than most people assume. A House of Commons Library briefing confirms there is currently no mandatory national licensing scheme for practitioners delivering treatments like this in England. The most reliable safeguard in the meantime is to check for accreditation through a body such as Save Face, which independently verifies training, insurance and clinical standards.
A few practical questions are worth asking before booking: which specific product will be used, and can the practitioner show it's CE-marked or otherwise regulated; how many Aqualyx treatments has this practitioner personally performed; and what's the plan if you experience an unusual reaction or complication. A confident, specific answer to each of these tells you far more about safety standards than a clinic's marketing photos ever will.
